Please call into surgery and bring with you
Proof of Address and Your Medical Card
or
Proof of ID and Address and a Registration Form which are also available from the surgery
Please Note You Will Not Be Fully Registered
Until You Attend For A New Patient Medical
We like to see all new patients when they register for a simple medical examination at which their general health can be assessed.
When patients move from one area to another their medical records often lag behind.
The registration medical is a valuable opportunity for you and your new doctor to meet each other and helps us to provide you with a faster and more efficient service.

It is estimated that there are 124,000 carers in the Lancashire area (1,300 are young carers which makes up 10.9% of the population) who look after a friend, relative or neighbour, unpaid and without support. We aim to ensure that all carers registered with the practice are identified and referred to Adult Care Services. You may print off the referral form, fill it out and bring it in with you on your first visit to the practice.
