If you move house within the practice area, please ensure that you let us know immediately (if in doubt regarding the practice area please contact reception for advice). If you move outside the area you should register with a doctor who is local to your new address. Many patients are changing to telephone companies other than British Telecom and many patients now have mobile phones. Please inform us of your new telephone number as soon as possible, so that we can update our records.
